Description
HYPERTENEVC indicates whether the person has ever had chronic high blood pressure/hypertension. This variable is created with supplemental programming by the IPUMS NHIS staff from the pre-1997 condition records. Defining Hypertension Records and Identifying Persons with Hypertension
For all available years, persons who were asked a direct question about having hypertension and who have a chronic hypertension condition record are given a "Yes" code for HYPERTENEVC. Persons who were asked a direct question about hypertension but do not have a chronic hypertension record are given a code of "No." Persons who were not asked a direct question about hypertension, or for whom it is unknown whether they were asked such a question, are given a code of "NIU" (not in universe), regardless of whether they have a chronic hypertension record.
1972 and 1978
For 1972 and 1978, hypertension records were defined as those condition records any of the following codes in the variable DCICDCOD in the original NHIS public use files:
- "4000" ("Malignant hypertension without mention of organ damage")
- "4001" ("Malignant hypertension with heart involvement")
- "4002" ("Malignant hypertension with cerebrovascular invovlement")
- "4003" ("Malignant hypertension with renal involvement")
- "4009" ("Malignant hypertension with multiple organ involvement")
- "401" ("Essential benign hypertension")
- "402" ("Hypertensive heart disease")
- "403" ("Hypertensive renal disease")
- "404" ("Hypertensive heart and renal disease")
These condition code values come from the International Classification of Diseases (ICD) Revision 8. Additionally, to be considered "chronic," hypertension condition records must have a code of "1" ("Chronic") in the variable CACODE in the original NHIS public use files.
1979 to 1981
Beginning in 1979, NHIS began using ICD Revision 9 to code conditions. For 1979 to 1996, hypertension records were defined as those condition records with any of the following codes in the variable DCICDCOD in the original public use files:
- "4010" ("Essential hypertension, malignant")
- "4011" ("Essential hypertension, benign")
- "4019" ("Essential hypertension, unspecified")
- "4020" ("Hypertensive heart disease, malignant")
- "4021" ("Hypertensive heart disease, benign")
- "4029" ("Hypertensive heart disease, unspecified")
- "4030" ("Hypertensive renal disease, malignant")
- "4031" ("Hypertensive renal disease, benign")
- "4039" ("Hypertensive renal disease, unspecified")
- "4040" ("Hypertensive heart and renal disease, malignant")
- "4041" ("Hypertensive heart and renal disease, benign")
- "4049" ("Hypertensive heart and renal disease, unspecified")
- "4050" ("Secondary hypertension, malignant")
- "4051" ("Secondary hypertension, benign")
- "4059" ("Secondary hypertension, unspecified")
As with earlier years, to be considered "chronic," hypertension condition records for 1979-1996 must also have a code of "1" ("Chronic") in the variable CACODE in the original NHIS public use files.
1982 to 1996
For 1982-1996, hypertension records were defined as those condition records with a code of "508" ("High blood pressure [hypertension]") in the variable C1CONDRC in the original NHIS public use files. This condition code value comes from NCHS's Diagnostic Recode C: Chronic Conditions and Impairments.
Weights
For 1972, HYPERTENEVC should be weighted with PERWEIGHT.
For 1978 to 1996, HYPERTENEVC should be weighted with CONDWT5.
Related Hypertension Variables
The nearest equivalent of HYPERTENEVC for 1997 forward is HYPERTENEV (Ever told had hypertension). However, researchers should use caution when comparing prevalence estimates derived from these two variables, as the manner of collecting information about hypertension differs greatly between the two. HYPERTENEVC was created by recoding condition records that were generated in response to several different survey questions about health problems underlying disability, sick days, or contacts with health care practitioners. HYPERTENEV, by contrast, is based on a direct question about whether the person had the condition hypertension.
Comparability
As with all of the person-level condition variables created by IHIS from condition records prior to 1997, the universe for HYPERTENEVC is substantially different for 1978 forward than it is for years prior to 1978. Beginning in 1978, only persons living in 1/6 of households, rather than all persons, were asked a direct question about ever having hypertension. This change requires that different weight variables be used depending on which years of data are analyzed. See the "Description" tab for details.
