For persons age 5 to 24 who need orthodontic care but have not obtained it (including persons who think they need to have their teeth straightened or who have been told by a dentist that they need to have their teeth straightened), ORTHNORM records the main reason why the person is not currently receiving care.

- 1974: Persons age 5 to 24 who have never had their teeth straightened or had bands on their teeth, and who think teeth need to be straightened or who have ever been told by a dentist or orthodontist that teeth need to be straightened.
