Description
For persons aged 64 and younger who have at least one assistive device that is not brace, artificial limb, hearing aid, or white cane and who used their assistive device at school or work in the last 6 months, ADWSPAIDRE reports if the person's device(s) used at school or work was/were paid for by a rehabilitation program.
