Description
For persons aged 64 and younger who have at least three assistive devices that are not braces, artificial limbs, hearing aids, or white canes and who used their third device at work or school and at home or somewhere else in the last 6 months, AD3SAMEHWS reports if the third device used at home or somewhere else is the same device that was used at school or work.
